Requirements:
In order for a High School student to be eligible to start their season, they must have all required WIAA items completed & be cleared through the high school athletic office. If you are not sure of your last physical, please check with Kelly in the High School Athletics Office now. Physical appointments can be difficult to schedule & are a non-negotiable; so please don’t wait until the last minute. *Please see the chart at the bottom of this page for physical expiration dates from the WIAA. Physical forms are available online via the High School website under Athletic Forms or available in the High School main office.
Students will not be able to practice until ALL required items for your sport are received & you have been cleared by the athletic office with your coach. **In addition, students with any outstanding school obligations (missing materials/equipment, uniform, school fees, etc) will not be eligible to start until ALL school fees, fines & obligations are taken care of.**
Please note that the items with one * should be completed online via the Annual Update/Online Student Registration process each year. Online registration for the 2024-25 school year will be available via your parent portal beginning July 10th, 2024. **The Annual Update/Online Registration is required for each school year; please log in to your Parent Portal & look for the Annual Update and New Student Reg link under the 'More' section. Please see this reference document for helpful hints to locate this in your parent portal.

WIAA Physical - current physical signed by a physician on file at school is required (see chart below)
- Physicals completed April 1-December 31 are good for the remainder of the existing school year & the following 2 school years (i.e. physical dated May 1, 2023 is good until June 2025)
- Physicals completed January 1-March 31 are good for the remainder of the school year & the following school year (i.e. physical dated March 3, 2023 is good until June 2024)
- Emergency Contact Information* - completed via the OLR Annual Update found on your Parent Portal under the More section; be sure to complete the Athletics portion of the OLR registration
- Concussion & Sudden Cardiac Arrest Waiver* completed via the OLR Annual Update found on your Parent Portal under the More section; be sure to complete the Athletics portion of the OLR registration
- Athletic Fee Payment of $80.00* - the athletic fee is $80 per sport/season & can be paid online or in the high school office
- Code of Conduct Video & Acknowledgement Google Form Submitted Annually
The Code of Conduct video for the 2024-25 school year will be available the week of July 8, 2024. The code video must be viewed by the student and parent. As students enjoy their summer months off, please remember that our Code of Conduct rules are in effect for all 12 months of the year and participation in extracurricular activities is a privilege, not a right.
