Sports Offered through the Mount Horeb Middle School

Please note sports with an asterisk(*) are only available to 7th and 8th grade students. 

If your child is interested in participating in Cross Country or Volleyball  this Fall, please contact call the middle school office 608.437.7306

Participation Requirements:
Before a middle school student can be eligible to participate in a practice or a competition, all of the following requirements must be on file in the office and a red card should be received by the student -athlete:
Athletic Forms
Physical Card (requires Dr. signature) or Alternate Year Card( Athletic information completed in OLR)
Concussion Waiver (Athletic information completed in OLR)
Code of Conduct Form(Video link) 
Athletic Fee Payment of $25.00 (This is required for each sport a student participates in during the school year.)
The Physical Card or Alternate Year Card and Concussion Waiver only need to be turned in to the office once a year.
Receiving information about The Code of Conduct Student Pledge is required each year.  The Code of Conduct video is available for the 2021-2022 school year.   Both student and parent/guardian must view the Code of Conduct video.
