Each year parents/guardians and students involved in a co-curricular activity or sport are required to be provided with information related to the code of conduct.  At the conclusion of watching the annual Code of Conduct presentation please sign the Code of Conduct form.  Both the student and a parent/guardian must sign the form.  Forms be found online or can be picked up in the Middle School office.  Signed forms must be on file in the office prior to a student participating in a practice or competition.
